Imipenem

About Imipenem

Imipenem is an injectable medicine used to treat severe, complicated bacterial infections inside the body. It is prescribed for serious conditions such as deep lung infections, severe urinary tract issues, and infections within the abdomen. This powerful carbapenem antibiotic is usually reserved for severe infections that have not responded to other antibiotics or are caused by resistant bacteria.

If you have been diagnosed with a serious bacterial infection, you may be feeling worried or overwhelmed. This medicine is prescribed to help control the infection and support your recovery.

Because this medicine is given directly into a vein or muscle, it must be used with consistent timing to keep an active, steady level of the medicine in your blood. It is typically injected multiple times a day under a strict schedule. While food does not affect how this injection works, drinking plenty of fluids, getting enough sleep, and eating a simple, balanced diet will greatly support your body's recovery.

While using Imipenem, some people may experience mild side effects like an upset stomach, mild diarrhoea, or soreness where the medicine was injected. However, you should contact your doctor immediately if you experience severe watery diarrhoea, a high fever, or signs of an allergic reaction like a skin rash or swelling. Your healthcare provider will monitor you closely during treatment to catch any issues early.

Do not take this medicine if you have had a severe allergic reaction to IMIPENEM, CILASTATIN, carbapenem antibiotics, or other beta-lactam antibiotics (such as penicillins or cephalosporins).  It is vital to talk to your doctor before starting this treatment if you have a history of seizures, brain disorders, or kidney disease, as these conditions may require special monitoring or a lower dose.

This medicine may interact with certain other treatments, such as anti-seizure medications, which can lower their effectiveness and increase your risk of health complications. You should also avoid drinking alcohol during your treatment, as it can strain your body and worsen side effects. Always consult your doctor if you are pregnant, breastfeeding, or managing other chronic health conditions to ensure this medicine is safe for you.

Uses of Imipenem

Imipenem is used in the treatment of Severe bacterial infections. The detailed use of Imipenem is as follows: • Treatment of Bacterial Infections: Imipenem is used to treat bacterial infections such as complicated urinary tract infections (UTIs), lower respiratory tract infections (e.g. pneumonia), intra-abdominal infections (e.g. peritonitis, abscesses), skin and soft tissue infections (e.g. cellulitis, wound infections), gynecological infections (e.g. pelvic inflammatory disease), sepsis and bloodstream infections, bone and joint infections.

Medicinal Benefits

Imipenem helps treat serious bacterial infections quickly and supports faster recovery. Key benefits of using Imipenem include:

  • Fast-acting treatment: By delivering the medicine directly into your bloodstream or muscle, it bypasses the digestive tract and starts destroying bacteria quickly, helping you recover faster.
  • Broad-spectrum antibacterial action: Imipenem targets and eliminates many different types of bacteria, making it effective against a wide range of serious bacterial infections.
  • Enhanced antibiotic effectiveness: The combination formula prevents your body's natural enzymes from breaking down the antibiotic too quickly, allowing it to remain active for longer and clear the infection more effectively.

  • Pain, redness, or swelling at the injection site
  • Mild skin itching or rash

Serious Side Effects (Call a doctor right away)

  • Severe, watery, or bloody diarrhoea accompanied by stomach cramps

Drug Warnings

  • Inspect the solution before use: Always check the mixed liquid before injecting it. If you notice any floating particles, cloudiness, or unusual changes in colour, do not use it.
  • Practice strict needle safety: Use a brand-new, sterile needle and syringe for every single injection. Never reuse or share needles, and discard them safely in a sharps container immediately after use.
  • Rotate your injection sites: If you are receiving this medicine as an injection into a muscle, change the injection spot each time to prevent tissue soreness and hard lumps from forming.
  • Monitor for secondary infections: Watch for signs of a new infection, such as white patches in your mouth (thrush) or intense vaginal itching, which can sometimes occur during long-term antibiotic use.
  • Complete the full course: Always finish the entire treatment plan prescribed by your doctor, even if you start feeling better. Stopping treatment too early may allow some bacteria to survive, causing the infection to return and making the bacteria harder to treat in the future (antibiotic resistance).

    Diet & Lifestyle Advise

    • Drink plenty of fluids: Staying hydrated is essential because it helps your kidneys process and clear this injected medication from your system safely.
    • Rest your body: Recovering from a severe bacterial infection requires a lot of energy. Prioritise sleep and avoid heavy physical exertion to help your immune system heal.
    • Eat easy-to-digest meals: Choose gentle, nutritious foods like simple broths, lean proteins, and cooked vegetables to keep your energy up without upsetting your stomach.
    • Consider gut health support: Strong antibiotics can temporarily disrupt your normal digestive bacteria. Ask your doctor if taking a high-quality probiotic or eating yoghurt after your treatment is appropriate for you.

    Special Advise

    • Imipenem may cause abnormal Coombs, liver, blood, and kidney function tests. Inform the person doing the tests that you are on treatment with Imipenem.

    Patients Concern

    Disease/Condition Glossary

Bacterial infections: A bacterial infection is a condition in which harmful bacteria enter, multiply, and infect the body. Harmful bacteria produce chemicals known as toxins, which can damage tissue and cause sickness. Symptoms of bacterial infection include fever, chills, and fatigue. Anyone can become infected with a bacterial infection. But, people with a weak immune system or taking immunosuppressive medicines are more prone to bacterial infection.

    FAQs

Imipenem is a combination of two drugs, namely: Cilastatin and Imipenem. Cilastatin inhibits the dehydropeptidase enzyme, which causes the breakdown of Imipenem, thereby helps imipenem to stay active for a longer duration. Imipenem prevents the formation of the bacterial protective covering, which is necessary for the survival of the bacteria. Together, Imipenem helps to treat bacterial infections.

Concomitant administration of Probenecid (used to treat gout and hyperuricemia) along with Imipenem is not recommended as it might result in an increase of plasma levels and half-life of Imipenem.

Ganciclovir (anti-viral) should not be co-administered along with Imipenem unless benefits outweigh the risks as generalised seizures have been reported when taken together.

Imipenem may cause changes in urine colour and amount of urine. Do not worry and consult your doctor if you have any concerns regarding this.

Imipenem may rarely cause thrush or candidiasis, which is a fungal infection occurring due to the overgrowth of yeast-like fungus in the mouth or throat. Regularly rinse your mouth with water to prevent the growth of fungus.

If you have had a severe, immediate allergic reaction to penicillin, you may also be allergic to this medicine. It is critical to share your complete allergy history with your medical team before receiving it. Talk to your doctor to find out if this treatment is safe for you.

This medicine is a parenteral drug, meaning it is injected. It is given either as a slow drip into a vein (intravenous infusion) or as an injection deep into a muscle. Talk to your doctor or nurse if you have questions about your specific injection method.

Imipenem is used to treat severe bacterial infections of the lungs, heart, bladder, skin, kidneys, bones, joints, blood, stomach, and female reproductive organs. It is an antibiotic and does not work against viral infections such as the common cold or flu. Talk to your doctor to determine whether your infection is bacterial and whether this medicine is appropriate for you.

Yes. Diarrhoea is a possible side effect of Imipenem. Drink plenty of fluids and eat non-spicy food if you develop diarrhoea. However, if you experience severe, watery or bloody diarrhoea, stomach cramps, or prolonged diarrhoea, do not take over-the-counter anti-diarrhoeal medicines on your own, as this may indicate a serious antibiotic-related bowel infection. Contact your doctor immediately for evaluation and treatment.

You should avoid drinking alcohol while receiving this medication. Alcohol can put extra stress on your body, cause dehydration, and make side effects like dizziness or nausea worse while you are recovering from a severe infection. Talk to your doctor or pharmacist for guidance.

Yes, this medication can sometimes cause seizures, particularly in patients who have pre-existing kidney problems, central nervous system disorders, or those receiving high doses. Your doctor will monitor your health and adjust your dose to minimise this risk. Talk to your doctor immediately if you experience any twitching or unusual neurological symptoms.

This medicine is typically reserved for pregnant or breastfeeding women only when there are no safer alternatives and the benefits clearly outweigh the risks. Talk to your doctor to discuss the safety of this medication for you and your baby.
